Just to clarify, Smart defence TTL scrambling will set all values to a
configurable value "default is 128" it does not change random ICMP
packets.
So far my money is on a load balancer...

I would sugest that the box you are tracing to is behind a Checkpoint
Firewall with Smart Defence enabled. Smart Defence has a TTL scrambling
ability to assist in the prevention of OS fingerprinting.
