CYA. Everytime. It doesn't matter how much you trust someone.
Things can change.
Always get a contract outlining exactly what will be occuring, what
you are allowed to do, who is authorising it, etc...
I couldn't find a template per se, but I did find an old post from the
Network Security archive which has some great guidelines:
